A simple PRR Analysis of COVID-19 vaccines
A list of safety signals, as announced by the CDC a long time ago.
Summary
For this article I performed a full proportional reporting ratio (PRR) analysis for COVID-19 vaccines based on US VAERS reports. This type of analysis helps researchers spot safety signals inside pharmacovigilance systems.
There were 1,926 hits for potential safety signals with reporting ratios of 1.1 or higher in reports for COVID-19 vaccines compared to reports for other vaccines.
The full list of results sorted by System Organ Class can be found in the Results section at the end of this article. I also supply links to CSV and PDF files which contain the results.

Introduction
On January 29th 2021 the CDC released a document in which they announced “CDC will perform Proportional Reporting Ratio (PRR) analysis (see section 2.3.1, p. 14), excluding laboratory results, to identify AEs that are disproportionately reported”.
They might have performed such an analysis, but the results were never presented to the public, which is why I decided to take up their job, albeit a bit late. I will even include the laboratory results.
I think that this extensive PRD analysis for various subsamples against age- and gender-adjusted pseudo-placebo reference groups is much more useful to both patients and doctors than a simple PRR, but we will start with the simplest version and then work our way towards a more complex analysis.
Methods
All my VAERS searches are run on the US subsample of the data due to inconsistencies in the reporting behaviour of foreign regulatory agencies.
Since I regularly perform age- and gender-adjustment on all subsamples in order to compare the distribution of side effects between the, I run a simple Regular Expression on the SYMPTOMS_TEXT field to determine the patient age in those reports that do not have the age fields populated by the CDC:

If the age can not be determined, the report is dropped.
I then split the dataset into two parts:
Study group: All reports about patients who received only a COVID-19 vaccine
Control group: All reports about patients who did not receive a COVID-19 vaccine
Now we count all occurences of every MedDRA term.
For every term we calculate the proportion of reports that contains them and calculate the proportional reporting ratio:
PR(study) = x(study) / n(study)
PR(control) = x(control) / n(control)
PRR = PR(study) / PR(control)
I then group the results by the following MedDRA System Organ Classes (SOC’s) with the help of bioontology.org:

I decided to move the Discussion section above the Results section since the latter is a list of 1,926 unique terms and their PRR’s of which many are listed in multiple categories which makes it a very long section.
Discussion
The PRR analysis you see in the article has the following weaknesses:
The age and gender structure of our two subsamples differs (solved by age and gender adjustment)
The PRR swallows information about the frequency at which a term occured in the two subsamples (solved by using the PRD instead, proportional reporting difference)
MedDRA is subject to changes over time
